Cooper Green Pharmacy (located in Jefferson County Department of Health) accepts prescriptions from Birmingham Health Care or other indigent providers but not other hospitals or doctors outside Cooper Green.
(B’ham Health Care has an application form for homeless persons.) There is a co-pay to see the doctor of $40-$60, depending on income.
Health First Cards - obtained at the Enrollment Clinic

• Residency in Jefferson County for 30 days or more
• Residency in Alabama for one year
• Current income
A letter from a homeless shelter can establish residency and income. Documents that show dates, such as a driver’s license (even an expired one), an I.D. card or an old utility bill, can be used to establish residency requirements.
Eye Clinic-routine eye exams; surgical procedures; no glasses dispensed.
Medicaid recipients must have referral from primary doctor, unless doctor is at Cooper Green.   Non-Medicaid recipients do not need a referral.

Seniors Eye Care Program: 1.800.222.3937 Ages 65 and older. Not have seen ophthalmologist in 3 or more years. Provides medical eye exam and services for a year.
Glaucoma Eye Care Program: 1.800.391.3937 Not had eye exam in 12 or more months. Qualification based on family history, race and age. Referral to ophthalmologist for screening and initiating treatment.
